Please call a specific
office for their actual hours. We are closed New Years
Day, Memorial Day, Independence Day, Labor Day, Columbus
Day, Thanksgiving Day (Thursday and Friday), Christmas
Eve day, Christmas Day and one-half day New Year's Eve.
What To Bring to an Appointment:
- A written prescription
for physical therapy from a physician.
- Current insurance
information.
- Comfortable clothing,
depending on the body part being treated.
Insurance:
As a courtesy to our patients, we file all insurance
claims, using our centralized billing system. Please
be aware that most insurance companies require an authorization/referral
for payment of physical therapy services.
We recommend that you verify your insurance benefits
for physical therapy by doing the following:
- Call your insurance
company to verify your physical therapy coverage.
- Investigate yearly
dollar coverage limits or number of approved treatment
visits for therapy and if pre-authorization for
treatment is required.
- If your condition
is related to a Worker's Compensation injury, you
should obtain information from your worker's compensation
insurance company, including the adjuster or case
manager's name, billing address and claim number.
